These are some big questions, but I'll take a stab at answering them, and I'm sure others will help out too.
1. There's no canon reason stated, so far as I know, as to why they're supposed to get permission before firing missiles. I would assume it's to emphasize the missiles' power and damage potential, and also to indicate that Dr. Nambu is in charge of the Team and that they aren't completely free to do whatever they want. Of course, fairly soon into the first series, they stop asking for permission to fire missiles and just do it anyway (though sometimes Ken still intervenes to prevent Joe from firing them) and presumably Dr. Nambu is okay with that. Of course, over time, the bird missiles cease to be as effective (something Joe frequently rants about, which leads to them getting "super bird missiles" in episode 67 -along with weapons for their personal vehicles). As for the firebird, everyone does act shocked and alarmed the very first time Ken suggests using it, but after that, although it's clearly very uncomfortable for them all, they seem to take it for granted as one more tactic/weapon at their disposal.
2. There's no precise canon answer for what causes Joe's health problems late in the series (like the ones seen in ep. 99). The doctor who examines Joe in ep. 103 talks about "an old wound" in his head and says "there are many bullet fragments still in there." This has been interpreted to mean the injuries Joe sustained when the Devil Star who killed his parents threw a rose bomb at him when he was 9, the shrapnel that he got in his head in episode 20 (maybe it didn't all come out at the end), or the injuries he sustained when the goons shot him up in episode 81 -or maybe even all three.
3. As far as I know, it was Popy, a toy company that also was a sponsor of Gatchaman. They wanted all new vehicles with an eye towards being able to sell new toys, though I would imagine that Tatsunoko might have also wanted a new look for a new series (ie. the Team's civvies were changed too and Popy wouldn't have had anything to do with that).
4. I'm not completely certain if the Gatchaspartan ever did the firebird. I don't think so, but there are others here who know more about Gatch F.

LB has pretty much answered most of it. I think for the New God Phoenix, it was part of the big re-designs they did when they released Gatchaman II. Keep in mind that they also redesigned the Team's civvies, their weapons, their individual vehicles, their base, the Snack Jun, ISO Headquarters, Sosai X, and the main Galactor villain.
As for the Gatchaspartan, it is my understanding that it was not capable of Firebird... until the final episode of the series (Gatch F 48 )... and as to why it could do it then... your guess is as good as anyone else's!

I have nothing else to add ... LB and TJ did a great job in answering the questions! As to the Gatch F series, I can confirm that, as also has already been said, the Gatchaspartan wasn't able to do the Firebird mode, at least as long as we, or even the same SNT, know ... during the series, the Firebird was only seen at the very end, under mysterious, unrevealed circumstances! As you have rightly said, Minerboh, we can say that the Hypershoot replaced the Firebird mode as the ultimate weapon.
